Blood Film

A blood film (peripheral blood smear) is a key laboratory technique used in Enfer to visually examine blood cell morphology and support the diagnosis of a wide range of haematological and systemic disorders. Blood film examination continues to be a vital diagnostic tool, even with the advancement of automated haematology systems. The peripheral blood smear enables direct visual assessment of blood cells, offering morphological detail and diagnostic insight that may not be fully captured by automated analysers. Blood film review therefore supports the differential diagnosis of patients with haematological abnormalities that may not be identified by routine automated methods.

At Enfer Medical, blood film review is included as part of the Full Blood Count profile and is further supported by digital morphology, with all relevant cases reviewed by our Consultant Haematologist to ensure accurate and clinically meaningful interpretation.

Clinical Details

When required a stained peripheral blood smear is examined to assess leucocyte, erythrocyte and platelet morphology and numbers. Increases and decreases in any of the normal leucocyte types or the presence of immature leucocytes or erythrocytes in peripheral blood are important diagnostically in a wide variety of inflammatory disorders and leukaemia or other haematological malignancies. Red cell and platelet morphology also alert clinicians to abnormalities such as hemolysis, anemia and presence of platelet clumping, and may even detect malaria/ A blood film is made and reviewed microscopically by trained staff if:

  • The FBC results meet the qualitative flags or quantitative blood film review criteria.
  • Is requested by a clinician. · A manual differential is required.
  • There are any other concerns whereby film review may be beneficial, e.g., the identification of storage artefact.

Some blood films may require additional review by the consultant haematologist. These films may be urgent or non-urgent. Blood Films that require immediate/urgent discussion with the consultant haematologist include those with the presence of blasts, schistocytes with thrombocytopenia, raised lymphocytosis, cytopenia and other criteria. Internal analysis is performed on a digital cell locator to facilitate remote examination by consultant haematologist. If required, the slide can be sent to the Consultant.

Transporting
Samples

In addition to the packaging of samples, the transport/delivery of clinical samples to the laboratory is required to be in adherence to all national regulations for the safe transport of biological materials. Enfer Medical provides a fully compliant logistics service, aligned with clients’ testing requirements.

Prompt transport of specimens:
To ensure prompt testing of samples and release of results within the published test turnaround times, samples should arrive to the laboratory by 18:00. Test turnaround time is measured from receipt of the sample at the laboratory until the time the authorised results are reported to users.

Acceptance/Rejection
of Samples

In rare cases specimens may not be suitable for testing on arrival to the laboratory. In that case the sample will be rejected at the receipt stage and the service user will be notified immediately and an explanation as to why the sample could not be processed will be provided. Reasons why samples cannot be processed include but are not limited to:

  • Samples received beyond the stability limits and/or not at the correct temperature indicated for each test.
  • Incorrect sample type received.
  • Leaking samples, sample not received or sample insufficient for analysis as stated below within sample requirements.
  • Non-compliant samples or request forms i.e. those missing sample date information, missing sample test request and/or missing sample site/type information.
  • Samples received without the necessary patient identifiers.
Clients will be provided with guidance on Sample Receipt Anomalies.

Result Values:
Following receipt at the Laboratory, results should be available within 4 working days and will be reported to the referring clinician via the Enfer Medical Client Portal or in a format agreed with clients. Established clients can login to the portal using the link above.
